My experience with prostate cancer

It all started with a classic PSA screening.  Urged by relatives and my doctor (French GP), I went ahead and did the test.  The result was borderline with PSA levels of 3ng/ml.  Just in case, my doctor recommended I scheduled a visit with a specialist.  I ended up seeing a urologist in a private hospital in Trappes, West of Paris.  This urologist lacked empathy and just saw me as a number in his busy schedule: very quick digital rectal examination (DRE) and jumps straight to the conclusion that a biopsy needs to be performed and subsequent prostate removal surgery if applicable.

I decided against it and did nothing in the short term.

A year later, I went for a follow up PSA test.  The levels rose has soared to 18ng/ml, clearly indicating high risk of prostate cancer.  My GP prescribed a prostate MRI, which indicated a suspicious stain on the left lobe of my prostate.  As a result, he sends me to see another urologist, this time at the Franciscaines Hospital in Versailles.  This urologist had a much better approach and took the time to explain my situation clearly to me.  This time I accepted to do a biopsy, which confirmed malign cancer cells with a Gleason score of 6 (3+3).  The urologist recommends the full prostate removal surgery.  My case is presented to the hospital’s commission which confirms diagnosis and treatment.  We therefore make an appointment for surgery.

But then I started thinking and realized that this commission is com posed mainly by urologists, radiotherapists and surgeons.  They are both judge and party.
Another issue was that they decided to perform an open surgery (“on the open sky” was his term) when there is a Da Vinci robot that allows for a less invasive surgery.  Why wasn’t I offered the choice?

Meanwhile, I tried to get more information through my GP and internet.  There, I come across the urologist lobby in France, the scandal of abusive prostate ablations, pseudo-scientific communications orchestrated by companies specialized in marketing for the benefits of major healthcare businesses, statistics showing that mortality remains almost the same whether or not you undergo surgery, the significant increase of prostate removal surgeries in france, from 7,000 to 28,000 per year in the last 25 years without having any significant impact on life expectancy but still affecting your quality of life with impotence and incontinence…  

And above all, the French high authority of health recommends not to practice routine screening.

There was a lot of food for thought and I decided to cancel the surgery.  To my surprise, no one from the hospital called me to make me change my mind or understand my decision.

In Bangkok, Thailand, I was told about HIFU (High Intensity Focused Ultrasound).  I view it as quite minimally invasive, with side effects greatly reduced compared to other treatments.  Although originally developed in France, French medical authorities still consider this treatment as experimental and do not use it as a first line treatment option.  Yet this method is about 17 years old and has been used more and more in the last 5 years in western countries.  The Samitivej Sukhumvit Hospital in Bangkok is the only hospital in Thailand to offer HIFU and has been using it for the last 18 months.

Passing through the rectum, the prod sends a focused beam of high intensity ultrasound, a bit like a magnifying glass which concentrates the sun’s rays, on the part to be treated.  The cells heat up and liquefy, and are then evacuated by urine.  The prostate stays in place and the risks of impotence and incontinence are very limited.

Why no urologist ever mentioned to me about HIFU in France is beyond me.  Especially when considering the fact that it leaves all other choices open in the event of recurrence of cancer.

So I took an appointment with the urologist at Samitivej, Dr. Isares.  After reviewing my situation, he explained to me the HIFU method, advised me to do a resection of the prostate first and then HIFU treatment immediately after.  Everything passing through natural channels, thus without incision of the abdomen as with conventional open surgery.
I was admitted to the hospital on Monday, March 17, in late afternoon, treated Tuesday morning, sent to ICU Tuesday early afternoon, went back to my normal room on Wednesday and left Samitivej on Thursday.

I was struck by the quality of care that takes into account the medical pain as well as the person.  We are Saturday, I am at home and feel no pain or hindrance, except for the urinary catheter that will be removed next Tuesday.


I would like to thank Dr. Isares for his fantastic patient care and the great job he did, Dr. Sombat the anesthesiologist who was always by my side during surgery with epidural block, the nursing team and all the many “small hands” that made my short stay at Samitivej very pleasant.

Pesto thaïlandais - Thai pesto

Avoir Makro à portée de scooter est une excellente chose et simplifie les courses. Malgré tout, le packaging de Makro est plutôt orienté catering et, pour un poignée de feuilles nécessaires à la réalisation d'un plat, il en reste les 9/10èmes  du paquet.
J'ai donc fait ceci:
1 sachet de noix de cajou, rescapé d'un précédent apéro :-)
Les feuilles de basilic thaï qui me restaient
2 gousses d'ail
Du parmesan
Et bien sur, de l'huile d'olive
Le tout est passé au mixeur jusqu'à l'obtention d'une pâte de bon aloi...
Une autre utilisation consiste à hacher grossièrement au couteau le basilic thaï puis à le mélanger à une huile végétale neutre. Cette préparation pourra être utilisée telle quelle pour réaliser le fameux poulet au basilic...

Having Makro in your neighbourhood is a good thing and simplifies the shopping. Nonetheless, Makro's packaging is mainly orientated catering and, for a handfull of leaves needed for preparing one dish, it will remain the 9/19th of the package.
So I did the following:
1 bag of cashew nuts, survivor of a previous drink :-)
The remaining basil leaves
2 cloves of garlic
Parmesan
And olive oil
All is passed to the mixer until you get a paste of good quality...
Another use is to coarsely chop the Thai basil with a knife and then mix it with a neutral vegetable oil. This preparation can be used as is for cooking the famous chicken with basil...

Un petit bijou niché dans la nature - A little gem nested in the nature

Grâce à la recommandation d'amis, nous avons déniché un véritable petit joyau niché dans la nature.

Un simple champ rectangulaire a été transformé en un petit paradis sur terre. Je ne suis pas écologiste dans le sens politique du terme, mais j'ai la conscience de tous les petits gestes du quotidien pour préserver notre monde.

ASITA Eco Resort est basée sur cette prise de conscience. Les matériaux utilisés sont proches de la nature : bois brut, pierres, terre cuite, plantations, une rivière artificielle avec son écosystème... Dans ce domaine, vous avez seulement des ampoules basse consommation, la piscine est sans chlore mais utilise de l'eau salée.

C'est un havre de paix situé à seulement 1 h 30 de l'agitation de Huahin ou Bangkok. Il est assez difficile de réaliser que vous êtes si proche de grandes villes.

Le personnel bien formé est amical et efficace. Les chambres sont spacieuses, bien conçus et confortables et le restaurant propose un large éventail de la bonne nourriture.

La proximité des attractions touristiques majeures, comme le marché flottant de Damnoen Saduak (pour les touristes occidentaux, mais vous devez le visiter au moins une fois), Amphawa marché flottant (mon préféré parce que de loin le plus authentique, avec très peu de visiteurs occidentaux mais beaucoup de Thaïs), les lucioles d'Amphawa, le marché de Maeklong avec le train qui le traverse, font de ce domaine un endroit incontournable.

En bref, un superbe week-end pour un prix très raisonnable. Je ne peux que recommander cet endroit et bien sûr, j'y retournerai.


Thanks to the recommendation of friends we unearthed a true little jewel nestled in nature. A simple rectangular field was transformed into a small paradise on Earth. I'm not green in the political sense of the term, but I have a consciousness of all small gestures of everyday life to preserve our world. Asita Eco Resort is based on this awareness. The materials used are close to nature: raw wood, stones, Terra cotta, plantations, an artificial river with its ecosystem... In this field, you only have low consomption bulbs, the pool is without chlorine but uses salt water. It is a haven of peace located just 1½ hours from the hustle and bustle of Huahin or Bangkok. It is quite difficult to realize that you are so close from major towns. The well trained staff is friendly and efficient. Rooms are large, well designed and comfortable and the restaurant proposes a wide range of good food. The close proximity to major tourist attractions, like the floating market of Damnoen Saduak (for Western tourists but you have to visit it at least once), Amphawa floating market (my favorite because by far the most authentic, a very few westerner visitors, but lots of Thais), fireflies of Amphawa, Maeklong Market train, make this resort an essential place. In short, a superb weekend for a very reasonnable price. I can only recommend this place and for sure, I will return.

Le permis de conduire Thaï - Thai driving license

Posséder un permis de conduire Thaï, c'est se faciliter la vie au quotidien. En effet, il tient également lieu de pièce d'identité, il porte votre numéro de passeport, il est au format carte de crédit ainsi il se glisse facilement dans un petit porte feuille et vous évite de vous trimballer avec le permis national, le permis international et le passeport. Sans compter qu'en Thaïlande, il n'y a qu'un permis moto qui est aussi bien valable pour les 90 cm3 comme pour les grosses cylindrées. Vous pouvez donc dépasser le seuil des 125 cm pour prendre un scooter en 150 voir 300 cm3.

Comment procéder ?

Munissez vous des documents suivants :

- Yellow book de votre logement
- Permis international
- Passeport

Allez au bureau de l'immigration qui est situé sur la route du canal et une des charmantes jeunes filles qui sont à l'accueil, à l'extérieur, vous vous remplir le formulaire ad'hoc, vous guider dans la démarche (photos d'identité, dépots des formulaires...), puis vous n'aurez plus qu'à vous acquitter de la somme de 800 THB par personne et attendre qu'on vous appelle avec votre demande de permis dûment signée et tamponnée.

Allez chez un médecin qui vous établira un certificat médical pour 60 THB. Là, vous êtes parés pour aller au bureau des permis.

A Huahin, on vous proposera Cha-Am ou Pranburi.

A Pranburi, il y a beaucoup d'étrangers et le personnel y est beaucoup plus tatillon et vous impose des tests psychomoteurs (Lumière verte, bouton vert, lumière rouge bouton rouge, vision périphérique, etc...).

A Cha-Am, bureau beaucoup moins fréquenté, on se contentera de "transformer" votre permis national ou international en permis thaïlandais. Là encore, vous aurez à faire sur place des photocopies de vos documents à 10 THB la copie. On vous établira un dossier que l'on vous demandera de signer. Puis, dans la pièce d'à côté, on vous prend en photo et votre permis au format carte de crédit sortira de la machine.

Et voilà.

Pour vous rendre au bureau des permis de Cha-Am, quittez Huahin par le nord, allez jusqu'à Cha-Am que vous traverserez et suivez Petchkasem en direction de Petchaburi. Ce sera le 2ème U-turn après le par Santorini Waterpark. Revenez sur vos pas et prenez la route située en face du magasin 7/11. Le bureau sera à 100 m sur votre gauche.



Owning a Thai driver license will facilitate your daily life. Indeed, it is an ID card too, it carries your passport number, it is in the format credit card so it fits easily into a small wallet and you don't you lug around with national license, international license and passport. Not to mention that in Thailand, there is only a motorcycle permit which is also well valid for the 90 CC as for large displacements. So, you can exceed the threshold of 125 cm to take a 150 scooter or why not a 300 cm3.

How to proceed?

Locate you on the following documents:

-Yellow book of your accommodation
-International permit
-Passport

Go to the immigration office which is located on the route of the canal and one of the charming girls who are outside, will fill in the relevant form, guide you in the process (passport photos, deposits of forms...), then you just have to pay the sum of 800 THB per person and wait until they call you with your application duly signed and stamped.

Go to a doctor that will establish you a medical certificate for 60 THB. There, you are ready to go to the permit office.

At the immigration office in Huahin, they will ask you to choose between Cha - Am and Pranburi.

In Pranburi, there are a lot of foreigners and the staff is much more fussy and requires you psychomotor tests (green light, green button, red light red button, peripheral vision, etc...).

At Cha - Am, the office is less overcrowded, it would confine himself to "transform" your national or international licence in a Thai driving licence. There again, you'll have to make photocopies of your documents on-site at 10 THB a copy. You will get a form that you will have to sign. Then, in the next room, a lady will takes pictures of you and your permit in format credit card will be printed out.

Et voila.

To get to Cha - Am permit office, leave Huahin heading north, go to Cha - Am that you will cross and follow Petchkasem in the direction of Petchaburi. This will be the 2nd u-turn after the by Santorini Waterpark. Retrace your steps and take the road opposite the 7/11 store. The office will be 100 m on your left.

La soi Moobaan Borfai s'anime - Soi Moobaan Borfai comes alive.

La soi Moobaan Borfai est dans le prolongement de la soi 6, après le pont du canal en prenant à droite. Quand je suis arrivé à Huahin, il y a 6 ans, la soi Moobaan Borfai n'était qu'un aimable chemin de terre rouge, bordé de quelques maisons. Aujourd'hui, le changement est total.

Tout d'abord, le chemin de terre a été totalement recouvert de bitume ce qui a permis une circulation plus agréable et, par voie de conséquence l'arrivée de commerces.

Ainsi, 3 pharmacies se sont installées et on y trouve tous les médicaments courants pour soigner les petits bobos de la vie quotidienne.

Pour les amis des bêtes, une vétérinaire s'est installée soi 6. C'est une jeune femme qui adore les animaux, j'ai pu le constater en lui amenant une petite chienne dont j'avais la garde.

Les vendeurs des rues sont pléthore et l'offre y est sympa : cela va du porridge de riz ou la soupe de nouille aux brochettes les plus diverses et bien sur tout cela est servi dans des sachets en plastique.

Fruits de saison, bananes, ananas, essence au litre, charbon de bois... Tout est à portée de main pour une somme dérisoire.

Un marché a lieu tous les lundi.

Un 7/11, ainsi qu'un Family mart, magasins de proximité ouvert 24/7, se sont implantés.

Les restaurants sont nombreux et offrent des plats pour tous les goûts.

Makro supermarché de gros pour professionnels et particuliers est juste à côté, soi 4.

Villa Market, le Fauchon thailandais a ouvert récemment un supermarché où l'on trouve nombre de produits européens.

Et si je vous dit qu'en plus, un hôtel ainsi qu'un cours de Yoga viennent compléter l'offre, vous comprendrez pourquoi je l'aime cette soi.

The soi Moobaan Borfai is in continuation of the soi 6, after the canal bridge taking right. When I arrived at Huahin, 6 years ago, soi Moobaan Borfai was only a friendly red dirt road, lined with a few houses. Today, the change is total.

First of all, the dirt road was totally covered with bitumen which allowed circulation more pleasant, and by implication the arrival of shops.

Thus, 3 pharmacies have settled and there are all common drugs to treat the small sores of everyday life.

For friends of animals, a veterinarian is installed soi 6. It is a young woman who loves animals, I've been able to check it when bringing a small dog I guard.

The street vendors are plethora and the offer is nice: Rice porridge or soup noodle for your breakfast, skewers of any kind, sauteed rice... all this is served in plastic bags.

Seasonal fruits, bananas, pineapple, petrol per litre, charcoal... Everything is at hand for a paltry sum.

A market is held every Monday.

A 7/11, as well as a Family mart, open 24/7 convenience stores, are available.

Restaurants are plentiful and offer dishes for all tastes.

Makro wholesale to professionals and individuals supermarket is right beside, soi 4.

Villa Market, the Thai Fauchon recently opened a supermarket where number of European products.

And if I told you that and more, a hotel as well as Yoga classes complement the offer, you will understand why I like it this soi.

Après une période de sommeil, période pendant laquelle nous étions en France, le blog se réveille avec notre retour au Pays du sourire.

J'aimerais vous parler d'électro-ménager. Quand on investit dans une maison ou un appartement neufs, il vient le moment ou il faut choisir son électro-ménager: four, micro-onde, réfrigérateur... Lors de l'achat de cette maison, il nous avait été conseillé de prendre du Teka, marque auréolée par le made in Germany.

Je vous le dit tout net, n'achetez jamais cette marque. Tous ceux qui l'ont fait dans le lotissement s'en sont mordu les doigts : grill du micro-onde qui refuse de fonctionner plus de 8 minutes, réfrigérateur censé ne pas givrer qui se transforme en banquise. Manifestement du bas de gamme absolument pas prévu pour fonctionner dans un pays chaud et humide.

Le seul conseil que je puisse vous donner est d'opter comme je viens de faire pour une marque japonaise ou coréenne. Ils savent faire des produits adaptés aux climats tropicaux.

When buying this House, he had been advised us to take the Teka brand, mark nimbed by the made in Germany.
